The Advalife Science Specific Certified Committee for Regenerative Medicine (the "Committee") and its founder, Advalife Science (the "Association"), comply with the Act on the Protection of Personal Information and other applicable laws, and manage the personal information handled by the Committee appropriately.
Article 1 (Personal Information We Collect)
The Committee collects personal information in the following situations.
1. Applications submitted through the review request form Name of medical institution or company, job title, name of applicant, name in kana, email address, phone number, address, class of regenerative medicine, title of the treatment or research, and preferred review period
2. Enquiries submitted through the contact form Name of medical institution or company, name, name in kana, email address, phone number, and the content of the enquiry
3. Contact by phone, fax, email or post Names, contact details and other information provided in the course of such contact
4. Documents submitted in connection with review proceedings Information contained in provision plans for regenerative medicine and other documents required for review proceedings
Article 2 (Purposes of Use)
The Committee uses the personal information it collects for the following purposes. 1. Receiving review requests, preliminary consultations, contract procedures, and invoicing review fees 2. Conducting review proceedings under the Act on the Safety of Regenerative Medicine 3. Notifying review results, minutes, written opinions and other matters 4. Responding to enquiries and contacting you 5. Making reports, and preparing and retaining records, as required by law If we intend to use personal information beyond these purposes, we will obtain your consent in advance.
Article 3 (Provision to Third Parties)
The Committee will not provide personal information to third parties without your consent, except in any of the following cases.
1. Where required by law
2. Where necessary to protect a person's life, body or property, and it is difficult to obtain your consent
3. Where it is necessary to cooperate with a national or local government body, or a party entrusted by such a body, in carrying out duties prescribed by law, and obtaining your consent is likely to impede those duties
Article 4 (Publication Relating to Review Proceedings)
In accordance with the Act on the Safety of Regenerative Medicine and the committee regulations, the Committee publishes an outline of the process of its review proceedings by registering it with "e-Regenerative Medicine," the database maintained by the Ministry of Health, Labour and Welfare. Matters that are likely to impede the protection of personal information, the originality of research, or intellectual property rights are excluded from publication. The names, affiliations and titles of committee members are also published on this website and in the database above, as required by law.
Article 5 (Outsourcing)
The Committee may outsource all or part of the handling of personal information to the extent necessary to achieve the purposes of use. In such cases, the Committee exercises necessary and appropriate supervision over the contractor.
Article 6 (Security Measures)
The Committee takes necessary and appropriate measures to prevent the leakage, loss or damage of personal information and to otherwise manage it securely. Under Article 16 of the committee regulations, committee members and other persons involved in the Committee must not disclose, without legitimate reason, any confidential information obtained in the course of review proceedings. This obligation continues after they leave their position.
Article 7 (Retention Periods)
In accordance with applicable laws and the committee regulations, the Committee retains records as follows. 1. Books relating to review proceedings: 10 years from the date of the final entry 2. Records of the process of review proceedings, submitted documents, and copies of notifications: 10 years from the date on which the relevant regenerative medicine ceased to be provided 3. The regulations on review proceedings and the list of committee members: 10 years after the Committee is dissolved Personal information for which the retention period has expired is disposed of by appropriate means.
Article 8 (Requests for Disclosure)
If you request notification of the purpose of use, disclosure, correction, addition or deletion of content, suspension of use, erasure, or suspension of provision to third parties in respect of personal data held by the Committee, we will respond without delay in accordance with applicable law. Please direct such requests to the contact point set out in Article 10. We will respond after confirming your identity.

Article 11 (Revisions)
The Committee may revise this policy in response to changes in law or other circumstances. Any revision takes effect when it is posted on this website.
